Symptom narrative

Patient obtained her men B vaccine. RN then administered the HPV vaccine and immediately after, patient was mid sentence talking to the RN and collapsed forward. Patient was sitting on the bed at the time of the nurse was able to catch her unless her back. RN reported that patient looked blue/pale. She was nonresponsive for about 30 seconds. No tremors noted. NP entered the room as patient was coming to. No postictal state, incontinence. Patient reported overall not feeling well with some lightheadedness, dizziness. She had no warning to passing out. Patient did become tearful and anxious/scared. She was very pale upon awakening. Vital signs were stable. She then started to complain of shortness of breath and some chest discomfort. Once she was starting to feel slightly better, did try to drink juice and crackers but felt like she was having some difficulty swallowing with the crackers and started to feel nauseous. Patient did stay in the office for about 2 hours and monitored. It did take her quite awhile to feel slightly better but had still complained of a intermittent headache. Dizziness, generalized weakness. Shortness of breath had improved. Upon getting her labs drawn, patient did have a lot of anxiety and became tearful and shaky. No syncopal episode noted at that time. She did note some abdominal pain that started while in the office.
