Symptom narrative

Needle slipped out during administration due to patient struggling, and patient only received an approximate of 0.1 mL in left arm during attempt, with about only about 0.25 mL left in syringe. With the agreement of the parents and approval of Dr., another flu vaccine from the same brand and lot # was drawn up and administered in the other arm, this time without incident. Parents were very understanding about incident and shallow scratch (appr. 1 cm) on left deltoid.
