Symptom narrative

Kinrix was administered at 2 years of age vs recommended 4-6 years of age. Pt was not due for a 5th dose of DTaP nor a 4th dose of Polio at the time of administration, causing the patient to have to get and additional Kinrix on 11/18/2025 make patient up to date on vaccines and have a valid 5th dose of DTap and 4th dose of Polio. Mom was very upset about the additional unneeded vaccine.
