Symptom narrative

Patient experienced typical side effects of the vaccine (fever, body aches, swelling at the injection site) within a day of receiving vaccine, but the injection site reaction (swelling) continued until today. Patient continues to have a small knot at the injections site, although that has improved and gotten smaller. Patient also had to take two courses of antibiotics d/t infections at the injection site, doxycycline hyclate 100 mg tabs x5 days & clindamycin 300 mg x7 days; infection seems to have resolved.

Asthma, cervical spine disorder, B-12 deficiency, GERD, Muscle spasms, obesity, pain/migraine, peripheral edema, peripheral vascular disease, recurrent major depressive disorder, vertigo, vitamin deficiency
