Symptom narrative

Patient received the vaccination at 10:25am. Patient had syncopal episode at 10:30 where he hit his head on glass and was assisted by father to the floor. EMS called. Patient was pale but no visible injuries noted, patient denied any vision changes, headaches, pain, nausea, or vertigo. Vitals taken were 132/70 and pulse 68 at 10:32am. EMS arrived at 10:36, blood pressure was 100/55 pulse 90. EMS assisted patient up to chair where patient felt as if he was going to "pass out" again. He had another syncopal event; his blood pressure was 55/31 at 10:45am. EMS then placed in gurney and was taken via ambulance to the nearest Emergency Department.
