Only the adjuvant portion of Shingrix administered to patient; Only the adjuvant portion of Shingrix administered to patient.; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a 51-year-old male patient who received Herpes zoster (Shingrix) (batch number NR4T5, expiry date 26-APR-2027) for prophylaxis. On 05-NOV-2025, the patient received Shingrix. On 05-NOV-2025, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: Only the adjuvant portion of Shingrix administered to patient) and inappropriate dose of vaccine administered (Verbatim: Only the adjuvant portion of Shingrix administered to patient.). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK receipt date:14-NOV-2025 The Nurse reported that, the patient was administered only the adjuvant portion (diluent only) of Shingrix. Registered Nurse wanted to know next steps to take and if the patient needed to be revaccinated.
