Symptom narrative

gave a vaccine to an adult, and it happened to be the child dose; gave a vaccine to an adult, and it happened to be the child dose; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of adult use of a child product in a patient who received HAV (Havrix Junior) for prophylaxis. On 14-NOV-2025, the patient received Havrix Junior. On 14-NOV-2025, an unknown time after receiving Havrix Junior, the patient experienced adult use of a child product (Verbatim: gave a vaccine to an adult, and it happened to be the child dose) and underdose (Verbatim: gave a vaccine to an adult, and it happened to be the child dose). The outcome of the adult use of a child product and underdose were not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 15-NOV-2025 Reporter wanted to know if the patient needs to repeat dose, and if they could give the adult dose or if they could just give a second child dose to make the full adult dose. The batch number was not provided upon follow- up with the reporter.
