No additional AE/No PQC.; Medical assistant reports MMR II was administered to 3 patients following a TE. HCP did not want to evaluate the MMR II for stability, and stated she was already sending it back to VFC she did not need it evaluated. MMR II was administered on 10/27/2; This spontaneous report was received from other health professional and refers to a patient of unknown age and gender. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 24-Oct-2025 (conflicting information, also reported as 27-Oct-2025, the patient was vaccinated with Measles, Mumps, and Rubella (Wistar RA 27-3) Virus Vaccine, Live (M-M-R II), reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT). Lot #Z004811 was verified as a valid lot number, with an expiration date of 20-Feb-2027. The vaccine was administered for prophylaxis (dose, route of administration and anatomical site of injection were not provided). The vaccine had been administered following a temperature excursion, reaching 70 °F for a time frame of 3 hours and 15 minutes. It was reported that there had been previous temperature excursions, and the time frame above is cumulative (Product storage error). No additional adverse events were reported (no adverse event).
