Symptom narrative

pt received Men B ( bexsero) by PCP office on 11/15/2025. Pt presents to my office 4 days post vaccination with 10 cm b 10cm hard indurated, erythema and edematous area at injection site. Orange peel appearance to skin with tissue hot to touch. Significant pain at site of injection. No signs of anaphylaxis. pt treated in office with antihistamine and nsaids for pain. home prescriptions sent for same
