6-month-old WCC was vaccinated with Quadracel. Pt was due for Vaxelis. Called the manufacture office to report it. (withheld name) from the manufacture stated that the patient is good with dose. Pt doesn?t need another dose. Called MOP was made aware of vaccine error. Pt will return for 2nd flu shot by 12/17/25 and will receive HIB and HepB. VAERS report was also fill out.
