Symptom narrative

had received completed series of Hep B April-2002. Her employer recommended starting another series due to high-risk job (janitor) and length of time since she completed last series. this was approved through the state immunization nurse at the time. Was informed this was up to her when restarting the series 10-2025. Received the injection and approximately 5-7 minutes later started having some tingling in her face. Her friends returned her to public health office. I took her back to clinic- she reported some swallowing difficulties. BP 164/83, pulse 111. No coughing & respirations easy, non-labored. Given 0.3 ml adrenalin. Offered to call ambulance. declined. Able to ambulate without dizziness & gait steady. 2 friends took her to Medical Center with instructions to go immediately, which they reported to do so. phone call to Medical and gave report to nurse with the above information.
