Symptom narrative

Patient requested and was given MenQuadfi at Pharmacy on 11/18/2025. This shot was apparently not recorded in the electronic system; it does not appear in the patient's chart. On 11/19/2025, he went to another location and asked to receive a MenB vaccine. Instead, he was given a second dose of MenQuadfi. He needs both vaccines for school, has now received a double dose of one and no doses of MenB. The recordkeeping is inadequate. No symptoms so far except normal sore arm.
