Symptom narrative

Patient came in for a Influenza Vaccine. Medical Assistant attached needle to pre-drawn syringe, verified correct vaccine with co-worker. MA went into room to give vaccine, prepped patient arm with alcohol pad, placed needle in patient arm, pushed plunger to inject fluid and part of the fluid came out of the syringe and ran down patient arm. Amount of injection received is unknown. Patient aware of assurance and verbalized understanding.
