The patient received Kinrix instead of DTaP and Polio vaccines.; The patient received Kinrix at age of 2 months; This non-serious case was reported by a other health professional via call center representative and described the occurrence of wrong vaccine administered in a 2-month-old female patient who received DTPa-IPV (Kinrix) (batch number Y49BZ, expiry date 17-MAY-2026) for prophylaxis. Co-suspect products included DTPa (DTaP vaccine) for prophylaxis and Polio vaccine for prophylaxis. Previously administered products included Hepatitis B (child received a birth dose). On 28-OCT-2025, the patient received the 1st dose of Kinrix. On an unknown date, the patient received DTaP vaccine and Polio vaccine. On 28-OCT-2025, an unknown time after receiving Kinrix and not applicable after receiving DTaP vaccine and Polio vaccine, the patient experienced wrong vaccine administered (Verbatim: The patient received Kinrix instead of DTaP and Polio vaccines.) and inappropriate age at vaccine administration (Verbatim: The patient received Kinrix at age of 2 months). The outcome of the wrong vaccine administered and inappropriate age at vaccine administration were not applicable. The mother did not want the patient to receive Hepatitis B even though the child received a birth dose. This was the patient first dose of DTaP and IPV.
