Symptom narrative

Only the adjuvant portion of Shingrix administered.; Inappropriate dose of vaccine administered; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 53-year-old female patient who received Herpes zoster (Shingrix) (batch number NR4T5, expiry date 26-APR-2027) for prophylaxis. On 05-NOV-2025, the patient received Shingrix. On 05-NOV-2025, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: Only the adjuvant portion of Shingrix administered.) and inappropriate dose of vaccine administered (Verbatim: Inappropriate dose of vaccine administered). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK receipt date:14-NOV-2025 The Nurse reported that, the patient was administered only the adjuvant portion (diluent only) of Shingrix. Registered nurse wanted to know next steps to take and if the patient needed to be revaccinated. The reporter consented to follow up.
