Symptom narrative

by accident was given another shot on Saturday; This non-serious case was reported by a nurse via call center representative and described the occurrence of extra dose administered in a 5-year-old female patient who received Flu Seasonal TIV Dresden (Fluarix 2025-2026 season) (batch number 73BN2, expiry date 30-JUN-2026) for prophylaxis. Concomitant products included INFLUENZA VACCINE INACT SPLIT 3V (FLUARIX). On 15-NOV-2025, the patient received the 2nd dose of Fluarix 2025-2026 season. On 15-NOV-2025, an unknown time after receiving Fluarix 2025-2026 season, the patient experienced extra dose administered (Verbatim: by accident was given another shot on Saturday). The outcome of the extra dose administered was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 17-NOV-2025 According to reporter, the confusion was responsibility of the mother of the child, the sibling of the patient was supposed to get the shot, but at the end patient ended up getting a 2nd shot. The vaccine administration facility was the same as primary reporter. The batch number was not provided and a request for this information has been made.
