Symptom narrative

No symptoms were reported. No other AE; Medical assistant calling to report that a 12 month old at their clinic was concurrently administered VARIVAX and PROQUAD on 27OCT2025; they meant to give the MMR-II and VARIVAX but made the mistake of administering the PROQUAD instead of the MMR-II.; This spontaneous report was received from a physician assistant (reported as medical assistant) and refers to a 12-month-old patient of unknown gender. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 27-OCT-2025, the patient was concurrently vaccinated with Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX), 0.5 mL, once (lot number Y007955 has been verified to be valid for Varicella Virus Vaccine Live [Oka-Merck] [VARIVAX], expiration date 17-APR-2026; route of administration, dose number and anatomical site of injection were not provided) and with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), 0.5 mL, once (lot number Z011864 has been verified to be valid for Measles, Mumps, Rubella and Varicella [Oka-Merck] Virus Vaccine Live [PROQUAD], expiration date 13-JAN-2027; route of administration, dose number and anatomical site of injection were not provided) (Accidental overdose); both vaccines were administered as prophylaxis and reconstituted with sterile diluent (HOLLISTER-STIER STERILE DILUENT) (expiration date, and lot number were not reported). The reporter stated that they meant to give Measles, Mumps, Rubella and Varicella (Oka-Merck) (M-M-R II), but instead they mistakenly administered Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) (Wrong product administered). No symptoms or other adverse events were reported for the patient.
