Symptom narrative

Describe Event, Problem, or Product Use Error: Following the patient's physical appointment where the patient received MMRV vaccine, the parent returned to the office, reporting that the patient had developed hives. The patient was promptly brought in and evaluated by the provider. An EpiPen was administered, and the patient was subsequently transported to the hospital for further care.

Medical history

4-year-old male patient presenting with anaphylaxis status post Kinrix and ProQuad vaccines today. Per the patient after leaving pediatrician's office after receiving Kinrix and ProQuad patient began coughing and sneezing frequently with mild difficulty breathing, and noticed the patient developing a right-sided facial rash that was not present on departing the pediatrician's office. At that time the patient's mother returned to the PCPs office where EMS was called and the patient was found to have a rash and given Benadryl which she immediately vomited and was subsequently administered an EpiPen and taken to the ED for further evaluation. On route patient's mother endorses improvement in patient's overall symptoms, no further episodes of emesis and improvement of the rash. Epinephrine was given at 1:09 PM.
