Patient was given HEP B and RSV in the hospital under the name [withheld], when the patient came to the office the name was [withheld]. There was no State Shot Record under that name so the immunizations were duplicated. The patient received HepB on 12/30/25 and RSV on 01/15/2025 in the hospital then again on 02/07/25 she received both vaccines in our office. The patient had no adverse reactions and the mother was notified today when it was discovered that the patient had 3 State records all with different last names. The mother states that the patient had no adverse reactions at the time of administration of the second doses of the medication.
