Symptom narrative

Patient had 3 charts in system and when I went to enter TDAP vaccine that was administered today- I noticed all three charts. When I opened the other charts, I then realized patient had a Tdap on 01/10/2023, that was not recorded in all 3 charts. The child did not need todays dose of Tdap.
