Symptom narrative

During visit after vaccine administration of Flu and MenQuadfi, pt passed out with postural presentation. Both Flu and MenQuadfi administered in the left deltoid. Approximately 15 seconds after administration of MenQuadfi, child looked at Dad and states "Dad I feel dizzy". Dad is standing in front of pt as she is sitting on the side of pt table. I was at bedside as well. Immediately after stating she felt dizzy, child passed out falling forward into Dad's chest. I instructed Dad that child has passed out and we should lie her on her back with feet propped up. Dad cooperative and assisted me in lying child down. During the transition, Child flexed her left arm behind her back and flexed to a side lying position. No clonic movements. Pt was easily able to be moved onto her back. I used the stool to prop pt's feet up to improve central circulation. Once in position, pt's arms and wrists were still flexed to an outward position. Pt eyes were open and looking up toward the ceiling. Pt was not responding to verbal stimuli. During this time pt's breathing and pulse remained stable. After approximately 30 seconds, pt became alert and reoriented. Dad still at bedside assisting and providing comfort to pt. I asked pt her name, today's date, who was standing next to her and their name. Pt was able to correctly answer all questions. While Dad at bedside, I opened the door to inform the other nurse of what took place and she went to get the MD to evaluate pt. I remained in the room at bedside. Pt still lying on back with feet propped. Pulse oximeter put on pt and received a reading of SpO2 100% and pulse 60 BPM. MD and second nurse arrived at bedside. SBAR given. MD assessed, asked pt and Dad some questions regarding events and pt history. About 10 minutes later, SpO2 remained at 100% and pulse now at 88 BPM. [withheld name] RN arrived to the room where SBAR was provided. [withheld name] RN and I sat patient up slowly to assess her response. Pt sat up and complained of nausea and tingling sensation in the hands. Reassuring vitals again. Pt requested to lie back down due to nausea. Denies offer of water, juice, snack. Cold wash rag applied to forehead. Dr [withheld name] then arrived for further assessment. Postural changes assessed. Pt states nausea and tingling sensation improving. Reassurance and education provided to both pt and Dad. Dad denies in need of anything himself. Dad remained calm and cooperative. After pt was further assessed by Dr [withheld name], MD agreed safe to have child return home. Education given for next steps and increase in activity. Pt and Dad expressed understanding. I walked pt out to their car in the parking lot. Pt safely made it into car. Encouraged Dad to reach out to office for questions or concerns. Dad expressed understanding. VARS report will be completed regarding adverse event.
