Symptom narrative

At 11:10 AM patient fell to floor and hit right side of head, fainted. Clinic nurse [withheld name] and Dr. [withheld name] were called. Patient was immediately woken per Dr. [withheld name] and brought to room. Primary provider Ms. [withheld name] was then notified.

Vital Signs as follows: 11:10 AM - HR = 65 apical, BP = 91/57, O2sat = 98% 11:20 AM - HR = 81, BP = 89/55, O2sat = 97% 11: 43 AM - Fingerstick Glucose = 97 11:50 AM, vitals prior to discharge from clinic: HR = 92, BP = 94/58 Manual, O2sat = 98% Child was observed for 40+ minutes, questions and concerns were addressed with mother in detail by provider [withheld name], APRN and MD. VIS sheets were given. Child alert and oriented, speech clear, PERRL x2, no nosebleed, no ear drainage, full ROM to neck, ambulating well. no bogginess noted to scalp, no erythema or bruising noted to scalp. Denies headache, denies vision changes, denies dizziness prior to leaving clinic. Drank 2 cups of pedialyte here. Returned following day 11/20/25 for follow-up visit for syncope after vaccination, resolved.
