Product preparation error; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of intercepted product preparation error in a patient who received RSVPreF3 adjuvanted (Arexvy) (batch number 5n79z, expiry date DEC-2026) for prophylaxis. The patient did not receive Arexvy. The patient experienced intercepted product preparation error (Verbatim: Product preparation error). The outcome of the intercepted product preparation error was not applicable. Additional Information: GSK receipt date:11-NOV-2025 The reporter stated that during preparation of an Arexvy vaccine, a pharmacy technician accidentally pushed out half of the adjuvant from the syringe and was not able to prepare a full dose.
