got accidentally a Shingrix first dose; Shingrix given instead of Flu shot; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of inappropriate age at vaccine administration in a 41-year-old female patient who received Herpes zoster (Shingrix) (batch number 3E27G, expiry date 28-APR-2027) for prophylaxis. Co-suspect products included Flu unspecified (Flu vaccine) for prophylaxis. On 12-SEP-2025, the patient received the 1st dose of Shingrix. The patient did not receive Flu vaccine. On 12-SEP-2025, an unknown time after receiving Shingrix, the patient experienced inappropriate age at vaccine administration (Verbatim: got accidentally a Shingrix first dose) and wrong vaccine administered (Verbatim: Shingrix given instead of Flu shot). The outcome of the inappropriate age at vaccine administration and wrong vaccine administered were not applicable. Additional Information: GSK Receipt Date: 17-NOV-2025 Pharmacist mentioned that a 41-year-old patient who was not immunocompromised got accidentally a Shingrix first dose. When call was transferred to second line Medical Information Specialist, caller mentioned that patient wanted to get Flu shot but they got confused and administered Shingrix by error. The reporter consented to follow up.
