Powder mixed with sterile water; powder component (vial) of Penmenvy shot was mistakenly mixed with sterile water and administered to a patient; This non-serious case was reported by a other health professional via call center representative and described the occurrence of wrong solution used in drug reconstitution in a 16-year-old male patient who received Men ABCWY NVS (Penmenvy) (batch number 499J9, expiry date 08-OCT-2026) for prophylaxis. On 12-NOV-2025, the patient received Penmenvy. On 12-NOV-2025, an unknown time after receiving Penmenvy, the patient experienced wrong solution used in drug reconstitution (Verbatim: Powder mixed with sterile water) and inappropriate dose of vaccine administered (Verbatim: powder component (vial) of Penmenvy shot was mistakenly mixed with sterile water and administered to a patient). The outcome of the wrong solution used in drug reconstitution and inappropriate dose of vaccine administered were not applicable.
