Symptom narrative

Right arm emitted a stream of blood immediately after IM injection then started to swell to larger than a quarter . I administered 10ml of diphenhydramine liquid and told Patient to hold ice compress on arm in the consultation room immediately following the vaccination. Patient stayed in the consultation room for about 30 minutes while I periodically checked on her. I did call back later about 6:15pm and Patient says her arm was still swollen and warm to the touch but otherwise unchanged. She was going to take another dosage of benadryl tablets closer to bedtime .. I told her I would call and follow up with her later on this weekend
