Symptom narrative

This is the RN reporting as the patient is hospitalized, I do not have access to verify the name and type of vaccines but per the mother: Flu vaccine on 10/10/25 at pharmacy, Covid vaccine on 11/16 at pharmacy, and Hep B and Meningitis vaccines on 11/19 at his pediatrics office. Covid was Moderna, Lot 8146751 0.5ml IM in Left arm. On 11/20 night, he developed petechiae, on 11/21 blood blisters in mouth, presented to the ER and had Plts of 1, diagnosed with ITP.
