Symptom narrative

Pt arrived for flu and covid vaccine. Flu vaccine given at 1543, covid vaccine given at 1546. Pt up walking around and feeling anxious. At 1550 pt telling her mother she didn't feel well. Pt became pale and stated she was seeing stars. She started to fall, nurse caught pt and eased her to a bench. Pt flat with feet elevated. She never lost consciousness Color quickly returned to her face. VS at 1555 - 111/60-71 VS at 1600 - 116/56-79-16 VS at 1610 - 104/64-86 Pt reports feeling better and sat up, drinking juice and eating crackers. Then pt up walking around in the lobby and feeling good. VS at 1615 - 105/68-83. Advised mother that when pt needs shots or blood draws in the future to have pt lie down. Pt discharged ambulatory at 1618.
