Symptom narrative

Pt received vaccine in right arm on 11/18/25 at 12:31pm. Arrived to pharmacy on 11/23/25 around 12:15am complaining of adverse reaction to vaccine- showed pharmacist her right arm which was red, swollen and hot to touch around her bicep area extending from 1 inch away from inner elbow to about 2 inches from shoulder & wrapped halfway around the outermost part of the arm. Pharmacist recommended to take OTC Benadryl, use OTC hydrocortisone 1% cream, and keep a cold compress on area and it no improvement by morning of 11/25/25 to see a walk-in or PCP. Told patient we will report this event.
