Site: Pain at Injection Site-Mild, Systemic: Dizziness / Lightheadness-Severe, Systemic: Fainting / Unresponsive-Severe, Systemic: Pt was seated during vaccine, and passed out and hit head.-Severe, Additional Details: Pt mentioned he didn't eat the day of vaccine. Rph called 911, pt laid down with feet up until emergency services were able to assist.
