Systemic: Dizziness / Lightheadness-Medium, Systemic: clammy and said lost ability to see colors but did not pass out-Mild, Systemic: Visual Changes/Disturbances-Mild, Additional Details: Pt informed immunizer that he was dizzy. he asked if he could remain seated. she went to grab him water and I went out to assist. His mother ad I laid him down and I elevated his feet . I stayed with him until clamminess and dizzyness subsided. had pt remain seated on floor for few more min then helped him up into chair and had him sit and drink water til Mom determined he was steady enough to walk.
