Symptom narrative

Systemic: Chills-Mild, Systemic: Dizziness / Lightheadness-Mild, Systemic: Fainting / Unresponsive-Mild, Systemic: Flushed / Sweating-Mild, Systemic: Shakiness-Mild, Additional Details: Patient reported feeling faint minutes after receiving two vaccines. Patient provided with cold water bottle & remainded seated throughout experience. Patient was seated on bench when began feeling faint and slumping over when patient mother noticed and propped patient up. When RPh [withheld] was alerted patient was already / still conscious -- unclear if lost consciousness briefly or not.
