Site: Pain at Injection Site-Mild, Additional Details: Patient received a duplicate vaccine for her flu shot on 10/29. The initial flu shot was given to her on 10/24. The immunizer, {withheld name} did not see that she received the vaccine on 10/24 and gave it to her again on 10/29. The patient did not report any adverse reaction to the 2nd flu shot.
