Symptom narrative

Site: Pain at Injection Site-Severe, Site: Redness at Injection Site-Medium, Site: Swelling at Injection Site-Severe, Additional Details: Swelling, pain and redness at injection site, a few hours after vaccine was given. Patient returned to pharmacy to show pharmacist. Pharmacist observed soft-ball sized swellikng on left deltoid area: recommended ibuprofen and benadryl to help reduce pain and swelling. if swelling worsening, recommeded patient to see urgent care.
