Symptom narrative

pt received flu and shingles vaccine on 10/3/25. pt stated it hurt more than other vaccines in the past. pt stated he experienced normal soreness at first, but then the pain and immobility progressed, pt went to the (withheld) clinic roughly 2 weeks ago and urgent care at 11/17/25, md stated pt had an injury from IM injection and is irritation his left arm nerve and causing radiation down the arm, pt has a fu scheduled at a future date
