Patient received HPV vaccine and sat down in room with father while waiting for the nurse to update her vaccine record and prepare visit paperwork. After 10 minutes, the patient stood up to leave and while in the clinic hallway, fell and hit head on the floor. patient appeared to have lost consciousness for about 2 seconds but was able to stand back up with assistance after regaining consciousness and walked backed to the exam table.Vital monitored for 25 minutes and applied ice packs. Father remained at bedside. Patient denied vision changes but did have a slight headache. Patient is alert and oriented x3. Father instructed to notify patients PCP immediately, father verbally understood and agreed . Patient appeared to have recovered and was able to walk out of office without no issue after 30 minutes.
