Symptom narrative

There was no adverse event. This was an administration error, patient is 9 years old and received a DTap when he should have been given a Tdap. No symptoms, parent was contacted and notified of the error. She was advised that this dose will count in the series, and he will get his routine booster of Tdap at age 11 per routine.
