After the patient's annual well child check, MA administered patient's HPV shot (2nd) in the left arm and flu (had multiple prior) in the right. Patient was sitting at the end of the exam table and fell back on the table and became unresponsive. MA yelled for help. Additional staff came to the room. Upon entering, the patient was rigid on the exam table. No tremors noted. Episode lasted approx 2 minutes. Staff stood on either side of the table to prevent fall. Anaphylaxis kit and vitals monitor brought into the room. MD at bedside. Airway patient. Patient not responding to verbal stimuli, beginning to move arms, legs, and turn head side to side. EMS was called. Mom was present and called her significant other. Vitals taken and monitored. Patient monitored through postictal period. Patient answering questions. Alert and oriented x 4. Reports that she is really tired and arms feel heavy. Has not eaten yet today. Has some abdominal cramping. No urinary incontinence noted. Mom reports no history of injection reactions or anaphylaxis. There is a family history of epilepsy, but patient has no history of seizures. EMS arrived and report given. Report called to RN at ER. MD called report to ER provider. No medications were administered. Parents discussing going to the hospital via ambulance or private vehicle. Patient departing via wheelchair with parents.
