Symptom narrative

The Shingrix vaccine was administered to the wrong patient. Patient was informed about vaccine error, provided VIS on Shingrix vaccine, and monitored for 15 minutes. No adverse reactions observed or reported. The next day on 11/21/2025, a telephone call follow-up was made to the patient. Patient reported no adverse reactions.
