Patient presented to pharmacy in am of 11/21/2025 for HD flu vaccination. Administered routinely, patient counseled, patient denied VIS sheet, left pharmacy without incident. I, [name withheld], Pharm.D., received phone call from patient's lay-caregiver at approximately 1700 on day of 11/21/2025. I was advised that patient had severe chills, body aches, that patient and lay-caregiver feel is secondary to vaccine administration. Counseled patient and lay-caregiver on self-management of symptoms, common side-effects of vaccine administration, when to seek medical attention if clinical state were to deteriorate. Patient followed-up morning of 11/24/2025 to advise that he developed "seizure-like" symptoms and patient sought emergency care for management. To my knowledge, patient was discharged as outpatient from emergency department. Upon speaking to patient on 11/24/2025 patient also advised that symptoms have fully resolved without further incident.
