No additional AE; a 2 yr old patient received a 2nd dose of PROQUAD early (they are not due again until age 4); Medical Assistant called to report that a 2 yr old patient received a 2nd dose of PROQUAD early (they are not due again until age 4); and also an inadvertent extra dose of PEDVAXHIB today; This spontaneous report was received from a medical assistant and refers to a 2-year-old patient of unknown gender. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 17-NOV-2025, the patient was vaccinated with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD). The vaccine was reconstituted with with sterile diluent (Hollister-Stier STERILE DILUENT), (lot # and expiration date were not reported). This vaccine was received early, as it should had been administered again until age 4 (inappropriate schedule of product administration; product administered to patient of inappropriate age). On the same date, the patient was also inadvertently vaccinated with an extra dose of Haemophilus b Conjugate Vaccine (Meningococcal Protein Conjugate) (LIQUID PEDVAX HIB) (extra dose administered). Both vaccines were administered as 0.5 mL (strenght, lot # and expiration date, route and anaotmical site of administration were not reported) for prophylaxis. No adverse effects were reported (No adverse event).
