Vaccine stored outside recommended conditions and administered to patient; This non-serious case was reported by a other health professional via call center representative and described the occurrence of incorrect storage of drug in a patient who received Flu Seasonal TIV Quebec (FluLaval 2025-2026 season) for prophylaxis. Co-suspect products included DTPa-IPV (Kinrix) for prophylaxis, Herpes zoster (Shingrix) for prophylaxis and HAB (Twinrix) for prophylaxis. On an unknown date, the patient received FluLaval 2025-2026 season, Kinrix, Shingrix and Twinrix. On an unknown date, an unknown time after receiving FluLaval 2025-2026 season, Kinrix, Shingrix and Twinrix, the patient experienced incorrect storage of drug (Verbatim: Vaccine stored outside recommended conditions and administered to patient). Additional Information: GSK receipt date: 19-NOV-2025 It was reported the vaccines were kept at Temperature negative1.65 degree Celsius for 3 hours. After collecting the temperature excursion information. Medical assistant inquired recommendations in case the vaccines were administered. She stated that she was making a general consult and she did not confirm that a patient had been administered with any of these vaccines however she approved the follow-up from Safety. The reporter asked the recommendation if someone was administered the vaccine, or do they need to be revaccinated, The reporter consented to follow up.
