Patients wife told NP her husband would be coming into clinic to receive flu vaccine and had NP schedule her husbands appointment. Wife told NP ahead of time her husband was hard of hearing. NP saw patient waiting in waiting room to collect drivers license and insurance card and confirm he was here to receive flu vaccine. NP checked patient in and confirmed he was here to give flu vaccine. Afterwards NP noticed another bandaid. Patient stated it was from the pharmacy. NP then saw flu vaccine record given to him by the pharmacy. Reported this event to his wife who then said next year she would go with him to his appointment. He confirmed that he can get forgetful.
