Symptom narrative

Patient was seen on 08/18/2025 for her Annual Physical Examination. She was administered the Prevnar 20 Vaccine in the left arm intramuscular. Patient called on 08/20/2025 to report that there was swelling in the area where the vaccine was administered. She was instructed to follow up in the office or via televisit on 08/20/2025 in which she denied both. Patient walked into the office on 08/25/2025 with swelling, rash and pain of the area where the Prevnar 20 Vaccine was administered Patient was instructed to proceed to the nearest ER because there was no Physician available in the office at the time on 08/25/2025. Patient called to confirm the office that she had been to the ER on 09/03/25
