We went to [withheld] in [withheld city, state], an assisted living facility to have a flu clinic. Patient asked for a flu shot, high dose. When we went to bill it to his insurance we found out he had already had a flu shot in August of this year at [withheld name] Pharmacy, so he received 2 flu shots.
