Reason for Visit: Patient presented for school-required immunizations. Nurse verified system and Epic records. Patient due for HPV dose #1, Menveo, Tdap, and influenza vaccine. Nurse obtained vitals O2: 95% HR: 90bpm RR: 20 BP: 108/65 Vaccine questionnaires complete. No allergic reactions to previous vaccines or vaccine ingredients. Patient stated he does feel anxious about getting vaccines. Both provider and guardian stated its ok to give due to no past reactions. Vaccines Administered Today: Two vaccines administered HPV and Flu . Post-Vaccine Reaction: Immediately after injection, patient reported feeling dizzy. Vital signs obtained: O2: 100% HR: 88 bpm RR: 20 BP: 86/60 Patient denied headache or pain. Water and crackers provided. Provider Notification: Nurse notified provider. Provider instructed to have patient rest and recheck BP. Reassessment: BP rechecked after 15 minutes: 96/66. Patient and grandmother reported patient "got nervous when he saw the shot go into his arm." After resting, patient stated he felt better. At 20-minute recheck, patient denied dizziness, headache, chest pain, or other symptoms. Observation: Patient observed for a total of 30 minutes post-vaccination without recurrence of symptoms. Plan/Follow-Up: Patient scheduled with pediatric provider on 12/8 for BP recheck. ER precautions reviewed; patient and grandmother verbalized understanding. After Visit Summary (AVS) provided upon discharge. Discharge Condition: Patient discharged in stable condition, ambulating independently, symptom-free at time of discharge
