County Public Health was conducting a flu vaccine clinic within the child's school as an emergency preparedness exercise. There were 2 children with the same first name in first grade. The teacher sent down one of the children, when child was checked in at administration desk only her first name was given, the vaccinator (name withheld) verified child's first name and the flumist dose was given to the incorrect child. This child's parents did not register her or consent to the flu vaccine. A while later the correct child came to the flu clinic and was given her dose as requested through registration done by her parents. (name withheld) mother was notified of the incident.
