Symptom narrative

Patient received flu vaccine at 10:30am. Started having left facial tingling and numbness by 10:40 am. Then tingling/ numbness left arm. Patient notified the workplace clinic of her symptoms at 3pm. She was advised to go to Emergency room or return for evaluation. Patient returned workplace clinic and assisted by provider that instructed Patient she needed evaluation in Emergency Department. Patient declined ambulance. She was evaluated at [withheld] Health [withheld city] Emergency room.

CT scan head- ruled out acute stroke, CT angiogram of head and neck, labs, EKG all preformed 11/24/25. Patient was recommended for admission to hospital and declined. Patient left against medical advice from Emergency room.
