Symptom narrative

Patient came in with family to receive influenza vaccine. During assessment questions patient's father had stated no reactions to other vaccines. Flu shot was administered and when the bandaid was going to be placed patient fainted for around 10 seconds. She woke up when alcohol was placed around the nose.
