Influenza vaccine was ordered. Patient was given RSV vaccine in error by Medical Assistant. Provider informed. Phone call was placed to patient on 10/30/25 and patient was informed of the error. Patient was not happy. Patient called into office on 10/31/25 and informed she had vomiting and diarrhea the previous night and developed a rash and itchy palms on 10/31/25. Provider recommended evaluation if having an allergic reaction. Follow up call made to patient on 11/4/25 by clinical manager to check on patient following the error.
